Important For Our Customers
Required Forms & Receipts
Participation in all FORPD activities is contingent upon completion of certain forms, such as Waiver and Medical forms. The forms required for each class activity are noted at the top of the activity detail page for that activity on our SmartRec registration system. Forms must be completed by the parent/legal guardian and given to the instructor along with online receipts at the start of the first meeting of each activity.
Family Information
Please enter complete and accurate information for your family when applying for an FORPD online account. Falsifying of information, e.g., family members or ages, will result in being withdrawn from the activity in question, forfeiture of registration fees, and restriction from future online registration.
Refunds / Activity Cancellation Policy
Refund/transfer requests for classes and programs can now be made online. Depending on the nature of the refund, some may be pending approval while others may have a "no refund" policy.
